Dear 2010 Camp Families,

Below you will find all the forms needed for a great stay at camp. Please complete and send the forms in to us by May 1st. Keep a copy of all forms and send the "Extra's" form to camp with the camper.

It takes a while to have the medical form completed by a physician so please take care of scheduling an appointment as soon as possible. If you are a returning camper and had a medical form completed within the last two years, you are not required to complete another. We have it on file.

We match roommates in the second week of May and continue weekly up to the camp date. The first group of roommate matches have more time to learn about know each other and plan for a great decorating theme.

Please mail all the forms together (except for the flight form if you are purchasing your flight after May 1st). If you are sending through a service, please make sure we do not need to sign for it. They will put it through our secure mail slot.
